Admissions and Records Forms

Use the information below to determine how to submit your Admissions and Records form.
Some forms are available through the Mt. 黑料导航 Student Portal, while others are available using direct links.

Before you begin:

  • If you are a current student, have your Mt. 黑料导航 Student Portal username and password ready.
  •  If you do not have portal access, have your Social Security Number (SSN) and a
      valid contact email address ready.
  • Most forms require up to 2 weeks for processing after submission.
    • Students with Portal Access

      If you can log in to the Mt. 黑料导航 Student Portal,  you must submit your forms by clicking the links below, which can also be found under your card.

      This is the recommended method because some of your personal information will be automatically prefilled, helping ensure accuracy and faster completion.

    • Users and Students without Portal Access

      If you do not have access to the Mt. 黑料导航 Student Portal, you may submit your request using the direct form links below.
